To obtain records and information of the former Panama Canal Zone
| Birth and Death Records | For birth and death (B/D) records, include the following in
your request: Date, purpose of request, name at B/D, date of B/D, city and country of birth, signature of subject or guardian, address and telephone. For birth records, also include: Father's name, date and place (city or state and country) of birth, mother's name date and place (city or state and country) of birth, and any US Passport information. Fee: $20.00 first copy and $10.00 each additional. |
