_____Nebo Regular Chorus:    Tuesdays 2:15 to 3:00:  3rd, 4th, and 5th Graders
                                                    First Practice:  Tuesday January 9th
                                                    This group is for students who are not participating in ASP Dance or
                                                    ASP Drama.  (Nebo Chorus is a free program.  No ASP Fees will be charged
                                                    for students picked up by 3:15.)

_____ASP Dance and Chorus:     3rd Grade:  Mondays 3:00 to 5:00: First practice: January 8th
                                                        4th Grade:  Tuesdays 3:00 to 5:00: First practice: January 9th
                                                        5th Grade:  Wednesdays 3:00 to 5:00: First Practice: January 10th
                                                        (No Auditions Required for Dance)
                                                        (ASP fees of $6.00 per day must be paid for this program.)

_____ASP Drama:     4th and 5th Grade Auditions:  Monday January 8th, 3:00 to 5:00 PM
                                    (Membership by audition – limited to 30 students)
                                    (ASP fees of $6.00 per day must be paid for this program.)

Parent signature indicates consent for above child to be photographed and/or videotaped while in rehearsals or concerts and authorizes use of these pictures without charge for publicity purposes related to the promotion of the Nebo Chorus.  I also release Nebo Elementary School from all claims of any interest of any kind of image on behalf of both the undersigned and the child named above.
