Employment Application Requirements

Your Best Care selects candidates for employment using the following steps:
1) Completed Application
2) Personnel interview
3) Employee Reference Checks — Personal, Employment and Criminal
a. Criminal background checks may be conducted at any time
b. If you are convicted of a crime or disorderly conduct, the employee must immediately inform the Personnel office or the Office Manager of the conviction. Failure to do so will result in immediate termination.
4) Attend Orientation and completion of forms needed for employment
5) TB test results
6) Basic Skills Class
7) CPR and First Aid Certification both for caregivers and residential aides

Orientation

Applicants will pay for a state background check and a TB skin test. After the employee has worked weekly for 3 months, the background check and TB test fees will be reimbursed to the employee.
If an employee becomes inactive for various reasons and remains inactive for one year, the application process will begin again. If the employee has been inactive for less than one year, a meeting with the Personnel Manager will be required to reactivate. Your Best Care reserves the right to use discretion when reactivating employees.

Salary Administration

Your Best Care strives to maintain a fair and equitable relationship between both the wages paid for the type of work performed, and the rates for similar jobs in the community among similar companies. Discussing pay rates with other employees is grounds for termination.

Overtime

In-House and Contractual Employees
Your Best Care allows employees that are directly caring for our clients choose if they would like to work 40 hours or more. Your Best Care follows the Kentucky Caregiver Overtime Exemption rule stating that caregivers may work more than 40 hours if they choose at regular pay, not time and a half. Caregivers are not penalized for not working more or less than 40 hours. It simply is a personal choice to the employee.

Recruitment

Your Best Care is committed to recruiting and retaining excellent employees. The company will use various resources including, but not limited to, classified ads, referral programs, job fairs, nursing schools, and word of mouth for recruiting purposes.

Caregiver Referral Bonus

Your Best Care offers active employees in the caregiver and resident aide position the opportunity to earn a referral bonus. If you refer a candidate that is offered a position with Your Best Care and is employed for at least one month with two (2) positive caregiver evaluations, then you will earn a $25.00 (twenty-five dollar) bonus. If the candidate does not put your name on the application or mention to the staff during the interview process that you referred them and is hired, you will not be eligible for the caregiver bonus. If two employees refer the same candidate, the referral fee will be split between the two employees. Employees may refer as many excellent caregivers as they choose and each referral will be eligible for a bonus.

Re-Hiring

A former employee who has left employment with the proper notification may be rehired. Unless otherwise
required by law, rehiring decisions will be made at the sole discretion of management based on an evaluation of
the employee’s personnel record.

Jury Duty

All employees will be given time off for jury duty if you are called. Please give us as much advance notice as possible. Written documentation is required as soon as you receive notification. In the event, that you are temporarily excused from jury duty, you are expected to report to the Office Manager.

Voting

Your Best Care encourages all employees to vote. Employees are encouraged to take advantage of polling hours prior to the beginning or following the end of your workday. If this cannot be arranged, management will approve time off to vote either at the beginning or end of your workday, provided that you give at least one day’s notice

Leave of Absence — Military Leave

A leave of absence is time off in a non-pay status. Upon receipt of a formal written request for leave of absence from a regular, full-time employee, management will determine whether a leave of absence will be granted. (Also see Medical/Family Leave) granted. The employee is expected to request leave of absence with as much advance notice as possible. Leaves of absence will not be granted for periods less than two weeks in duration. Vacation or sick leave should be used for such absences.

Military or Personal The leave classifications are defined as follows:

Military—To protect the employment rights of employees entering the armed forces of the United States and to ensure conformance with applicable federal laws, a leave of absence to serve in the military on active duty, must be granted to full time employees.

You may take a military leave of absence under the following restrictions:

1. Initial training period in the National Guard;
2. Being ordered to active military service as a member of the Reserves or National Guard for an indefinite period or for a periodic training period up to ten working days; and Any service requirements under the Selective Service Act.

Personal Leave of Absence

Except for those situations covered under the Medical Family Leave policy, personal leave may be granted to employees having special nonmedical personal needs for an extended period of time. Each case will be evaluated on its own merits.

Family and Medical Leave

Team members under eligible for leave under the Family and Medical Leave Act (FMLA) are those who:
• Have been employed by Your Best Care for at least 12 months, and
• Have worked at least 1,250 hours during the 12 months immediately preceding the leave.

General Provisions
An eligible team member is entitled to a total of 12 weeks of unpaid family leave during a 12-month period for the following reasons:

1. Birth of a child or placement of a child for adoption or foster care.
2. Serious health condition of a spouse, child, or parent.
3. Team member’s own serious health condition.
Your Best Care requires eligible team members to use accrued vacation leave, personal leave or sick leave to substitute for unpaid FMLA leave taken for a team member’s own serious health condition or the serious health condition of the team member’s spouse, child, or parent.
